o   Paid online: send form back and write the order confirmation code on the form with child’s name. This will help ensure your child receives their pictures in a timely manner.

·         Want to purchase using cash or check?? Complete form attached to flyer and return sealed with payment before Feb. 29, 2024.

·         Each child must have their own envelopes with all the correct information pertaining to that child.

·         Packages range from $14.99 – $74.99.

·         We, West Side, will not make change!  Please send the exact amount!
